Office Visits:
As a courtesy to our patients we will submit your claim to your insurance company for you. All co-payments and deductibles are the responsibility of the patient and expected to be paid at the time services are rendered.
Materials:
All material orders (glasses, contacts, lenses, etc) require payment in full prior to the material being ordered.
We accept Visa, Mastercard, American Express and Discover credit cards. We also accept debit cards and flexible spending cards with the Visa or Mastercard logo on it. Additionally, we take CareCredit please click here to learn more
Word About Insurance
Insurance coverage is a contract between you and the insurance company. There are many different plans available therefore it is not possible for us to quote benefits to you. Please refer to your plan policy or contact your carrier directly for plan policy questions. It is your responsibility to verify that we are in-network providers for your plan and obtain any referrals that may be required.
Though we will bill your insurance company as a courtesy, the patient is ultimately responsible for any account balance.
